The person, or other identified entity, to which a health record, note or communication primarily relates.

#### Notes

* The *subject of record* is formally represented in the SNOMED CT concept model by the concept [410604004 | Subject of record|](http://snomed.info/id/410604004) . This is the default value of the [408732007 | Subject relationship context|](http://snomed.info/id/408732007) attribute that indicates a finding or procedure relates directly to the subject of the record rather than to another person such as a family member or disease contact.
* Usually the *subject or record* is also the subject of care.
